Subscriber provided template is imported into AML for Quote Export

Reseller property (by REALM) has a unique Quote template name - would be merged with the quote data.
The template will have velocity tags for each data element from the order object to be merged.

The template includes the following info in the header:
reseller logo (left justified)

{Subscriber Name} {Product Name}

(right justified)

{Subscriber Phone Number}

(right justified)
Prepared By:

{Order Last Mofified By Name}

and the Footer Text is:
THANK YOU FOR YOUR BUSINESS! (left justified) with Page# (right justified)

The body layout will appear as attached

The BA/Loc table is a one to one relationship with BA/Loc, so if Plumbers is sold for Providence and Newport you would see:
Plumbers Providence
Plumbers Newport

Grouped by BA and sorted alphabetically

The following 4 Field Names must display, even if there is nothing entered in the field:

Comments:
Paid Search Primary Destination URL:
Campaigns:
Total for all Campaigns - Monthly Budget:

If there is nothing entered for the other field names, then the field name is not displayed.
