SReporting3: Columns with identical names are counted only once, leading to blank columns labels returned by getUserReports

If the user has access to two accounts that use different column IDs but
identical labels, the hashmap used to bind ids to labels will only have a single
ID associated with the label.

Example in Prod and Dev:

Column ID: 19
shortDescrip: GM $
Account: 0

Column ID: 158
shortDescrip: GM $
Account: 289 (LegalZoom)

This example effects the result of getUserReports in such a way that any reports
that have column ID 19 will not be labeled 'GM $' if the user has access to
LegalZoom.
