Alerts: Alerts create screen fails to render if an account name has an apostrophe

On staging, create an account called Test's. Go to Alerts->Create/Edit after
choosing that account in the upper right corner of the list. The screen for
Create/Edit fails to render correctly. See attached screen shot.

Rename the account to Tests. Follow the same procedure. The screen renders
correctly.

This happens in both staging and in production. In production it was effecting
the TGI Friday's account which I renamed TGI Fridays to work around this.
