Directory-level appended params aren't identified, only page level ones i.e.
      http://www.opiatesrx.com/test.php?ph=866.353.2683 VS
      http://www.opiatesrx.com/?ph=866.353.2683

Mark Fillmore's email on this here detailing it, also see attached spider report:

      Appended Parameter Sorts
Appended Parameters not being sorted out. This is due to our rules engine not
compensating for improperly formatted parameters - that is, appended parameters
at a "directory level" as opposed to a "page level". There are many reasons why
we run into this quite often, clever coding by a client or sometimes a simple
poorly formatted internal link using relative pathing. Regardless, we need to
      sort these out.

      Example:
      http://www.opiatesrx.com/?ph=866.353.2683 - we don't sort (but should)
      http://www.opiatesrx.com/test.php?ph=866.353.2683 - we usually sort

      Plus Sign
Also noticed in this spider that we didn't sort out Plus Signs.
