Location extensions re-used after terminating order and recreating it.

The description of AML-3781 sounds like we may have a different bug than was being reported.

We should investigate if this is actually happening as described.

A terminated and redeployed order should use the new location extension information, not the information from the terminated order. It should also successfully deploy with no location extension configured if so desired.

Note : this might only manifest itself if an order goes to ERROR state, possibly only due to a failed location extension. Investigation should include these cases.
